Office Hours
Our
office is open from 8 a.m. to 5 p.m. Monday through Thursday and from
8:30 a.m. to 4:30 p.m. on Friday. Patients may reach us by telephone
during these hours by calling 362-EYES.
On your first visit, it is important that we have all the necessary
information pertinent to your exam. Please bring with you:
- Your completed patient information form (we will mail this when we
schedule your appointment)
- Your eyewear
- A list of current medications
- A copy of your current insurance card
- A copy of your medical records
A complete eye examination will include
dilating (enlarging) the pupils. This may cause temporary blurred vision
(especially for reading) and sensitivity to bright light. Some patients
experience difficulty in driving because of this. You may want to make
arrangements accordingly.
Fees and Insurance
Payment is expected at the time services are rendered for all office
visits. You may make your payment in cash, by personal check, by bank
debit card or by MasterCard, Visa or Discover Card. Financing options
are also available. If you have any questions about what these charges
will be, we will be glad to provide that information to you. Our billing
office will submit your medical claims to both your primary and
secondary insurance carriers.
